SUPPORT COORDINATOR

Hannah Sackett

Hannah has been part of the Utah DSPD world most of her life personally, and for over 12 years professionally, with younger siblings that receive support from this division.
Hannah understands how overwhelming it can feel to navigate through available support as she’s observed and experienced the personal side of care for individuals in group homes, assisted living and nursing facilities as well as in their homes through respite, personal assistance, and supported living models. She’s been employed through the self-administered model as a family member to individuals receiving DSPD support. She's also volunteered, interned, and worked with disability support groups, care facilities, playrooms, with behavior consultants, and at a holistic wellness center for neurodivergent children with behavior concerns. Now she is pleased to be helping others as a Support Coordinator with Evergreen Supports and Advocacy.
Hannah earned a bachelor's degree in Family Life with an emphasis on Human Development through Brigham Young University.
She is passionate about helping people best utilize the supports available to them while maintaining high quality, personalized care. Hannah is a powerful advocate for her clients and their care teams.
In her personal time, Hannah is a licensed massage therapist, providing massage care to her loved ones, is a novice backyard gardener with big landscape dreams, enjoys making food from scratch, spends time with her large family and takes lots of breaks to play fetch with her doodles.

From there she became a liaison between treatment providers and specialty court programs for the Utah State Courts. She is thrilled to provide you with quality support and advocacy so we can make sure your needs are met!
